Registered Nurse – Discharge to Assess Team
The Discharge to Assess team at Stockport NHS Foundation Trust is looking for a Registered Nurse to join their community-based team.
About the Role
We are seeking individuals who share our enthusiasm, embrace diverse working methods, and can work flexibly across organizational boundaries. A position has opened for a Registered Nurse – Band 6, to work within the Discharge To Assess/Intermediate Care at Home Team.
The Discharge To Assess Service
This multi-professional service assesses patients upon their discharge from hospital in the community. We provide individualized care to meet the person’s needs and facilitate their transfer to the appropriate service: Health, Adult Social Care, or partner agencies.
The Intermediate Care at Home Team
This team provides nursing and therapy rehabilitation to patients within the Stockport community.
Responsibilities
- Support people who are clinically optimized and do not require an acute hospital bed, but may still require care services with short-term, funded support to be discharged to their own home or another community setting.
- Assessment for longer-term care and support needs is undertaken in the most appropriate setting and at the right time for the person.
- Provide holistic multidisciplinary interventions and support to stabilize patients in their own home or usual residence.
- Incorporate a free, short-term rehabilitation service for adults to help prevent unnecessary hospital admission, enable early discharge from hospital, and prevent unnecessary admission to long-term residential care.

- Flexibility to cover all areas within the Discharge to Assess service, covering 7 days 08.00-21.00.
