Job Description: Registered Mental Nurse (RMN)
We are looking for an RMN who is highly motivated, innovative, enthusiastic, and passionate about patient-centred care to work within the Bridgend South CMHT.
The team works with a variety of complex patients but is supportive. There will be occasional weekend working and relevant training needs can be supported.
Responsibilities
- Undertake responsibility for the care coordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.
- Take a lead role in the care planning process, take on care coordination responsibilities, and work within the Care Programme Approach ensuring that Service and CTMUHB policies are adhered to.
- Take part in the evaluation of the care of individuals at review meetings, case conferences, ward rounds, etc.
- Provide specialist knowledge and advice to other agencies for the purposes of public protection.
- Attend relevant specialist assessment training and formulate to encourage safe and appropriate risk management.
- Attend all relevant mandatory training as and when required to do so.
- Undertake the necessary training and professional development to maintain the standards of professional practice and to meet the registration requirements of the NMC.
