Band 6 Community Psychiatric Nurse
30 hours per week, 9-5 (week days only - to be discussed)
We are seeking an enthusiastic and motivated Band 6 Mental Health Nurse to join our OCPU Community Mental Health Team within the Gedling & Hucknall area. We provide assessment, treatment and support for service users of any age with a diagnosis of a dementia, cognitive impairment or people over the age of 65 with a mental health condition.
The team provides specialist assessment, therapy & treatment, working to an individually tailored care plan to adopt a flexible, patient-centred approach enabling the older person with mental health difficulties to engage in services available to them and to lead as independent a life as possible.
Delivered in people’s own homes within the community, the service makes every effort to prevent unnecessary admission to hospital and facilitates the opportunity for people to return home following a hospital admission or period in residential care.
There will be an expectation that the Band 6 Clinician will be the duty worker for the team.

- To provide a defined level of specialist assessment and access to interventions for a diverse patient group and their families under the care programme approach, who present with significant mental health conditions and people with a dementia diagnosis.
- To provide support to patients within their own community setting.
- To demonstrate active clinical and managerial leadership skills within the team, providing education and supervision to junior staff members and other professionals.
