Are you a Mental Health nurse looking for a new and exciting challenge?
We are looking to recruit an enthusiastic and motivated band 6 mental health nurse to work with us in the South Gwynedd older persons mental health team. This is a part time 15 hours per week secondment post for 3 months based in Uned Meirion, Ysbyty Dolgellau.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The successful applicant will be willing to work flexibly within both our Day Assessment and Community Service, and be passionate about improving services for people with dementia. As care co-ordinator, the post holder will be working closely with other agencies such as social services to support people in the community, whether that be in their own home, in hospital, or in a care home.

The post holder is required to work within an integrated MDT which provides community service, day assessment and outpatient service for those with memory impairment and dementia diagnosis. The post holder will be responsible for co-ordinating client care by assessing client needs, planning and implementing the plan of care in accordance with the documents of the Mental Health Measure 2010. Also to provide a detailed psychiatric nursing assessment by participating in the discussions and allocating referrals to the CMHT, and to provide nursing needs assessment for the LHB and Social Services, as well as considering criteria for funding such as CHC, FNC and Local Authority. The post holder will work well within the MDT team, have good communication skills and provide supervision, guidance and support to junior members of staff.
