About the Role
The Trust successfully bid to establish a Rough Sleeping Team and we are now recruiting staff to a Multi - Disciplinary Team. An opportunity exists for a CPN to join this Team and play a key part in its development and response to the needs of rough sleepers who have mental health difficulties and or serious physical health challenges. The service works across the tri-Borough area of Hammersmith & Fulham, Ealing and Hounslow and works closely alongside our principal partners, St Mungos and Thamesreach Outreach Services.
This is an exciting development providing a much-needed service and in addition to your established professional skills you will be a good communicator with a degree of flexibility and willingness to work with a number of agencies. The Team, in addition, includes a Consultant Psychiatrist, Social Workers and an OT.

The post involves working shifts which enable us to access service users effectively in order to assess mental and physical health needs, engage successfully and pursue reconnection with health and appropriate accommodation services.
