A rare opportunity has arisen for a Community Specialist Practitioner within our friendly Community Integrated Care Team in Lower Lea Valley.
The Specialist Practitioner will work within the Integrated team of Community Nurses, Therapists, CPNs, and Social Workers. You will have responsibility for ensuring all health and social care needs are met to improve health outcomes, functional status, and quality of life. This will lead to increased patient choice and prevent unnecessary admissions to hospital and reduce the stay of necessary hospital admissions.
