The postholder will work as part of a community nursing service working across the Fylde coast, overseeing/supervising the provision of nursing interventions in patients’ homes or clinic settings.

COMMUNICATION

  • Communicate sensitive information
  • Demonstrate effective communication and documentation
  • Handle information/data in line with national and local policies and legislation

PROFESSIONAL

  • Display a personal commitment to professional standards and ethical practice
  • To assess, plan and evaluate individual patient requirements, implement treatment regimes and prescribe when appropriate in line with Trust guidelines
  • Facilitate achieving measurable standards of care and participate in clinical audit and caseload reviews

CARE DELIVERY

  • Deliver holistic, person-centred nursing care to housebound patients and clinic patients who fall under the agreed service specification
  • Ensure the rights of individuals are held and facilitate the resolution of any conflict
  • Safeguard and protect vulnerable adults and children

TEAM WORK AND LEADERSHIP

  • Support quality improvement and optimisation of resources to improve services and promote equity in access to services
  • Participate in appraisal process and Personal Development Plans

EDUCATIONAL

  • Undertake the role of mentor and preceptor
  • Encourage reflective practice, use of professional portfolios, MURRS documents, and critical incident analysis

RESEARCH AND DEVELOPMENT

  • Apply critical analytical skills in research/audit/service improvement context
  • Adhere to ethical, legal, governance and quality assurance frameworks that pertain to research development and innovation
