The postholder will work as part of a community nursing service working across the Fylde coast, overseeing/supervising the provision of nursing interventions in patients’ homes or clinic settings.
COMMUNICATION
- Communicate sensitive information
- Demonstrate effective communication and documentation
- Handle information/data in line with national and local policies and legislation
PROFESSIONAL
- Display a personal commitment to professional standards and ethical practice
- To assess, plan and evaluate individual patient requirements, implement treatment regimes and prescribe when appropriate in line with Trust guidelines
- Facilitate achieving measurable standards of care and participate in clinical audit and caseload reviews
CARE DELIVERY
- Deliver holistic, person-centred nursing care to housebound patients and clinic patients who fall under the agreed service specification
- Ensure the rights of individuals are held and facilitate the resolution of any conflict
- Safeguard and protect vulnerable adults and children

EDUCATIONAL
- Undertake the role of mentor and preceptor
- Encourage reflective practice, use of professional portfolios, MURRS documents, and critical incident analysis
RESEARCH AND DEVELOPMENT
- Apply critical analytical skills in research/audit/service improvement context
- Adhere to ethical, legal, governance and quality assurance frameworks that pertain to research development and innovation
