Community Staff Nurse Opportunity

An exciting opportunity has arisen for the recruitment of a Community Staff Nurse based at Lytham District Nursing Team. The advertised role is a permanent, full-time position (37.5 hours per week).

It is essential that you have a full UK driving licence and access to a car for this Community Nursing role.

If you are innovative, enthusiastic, have a positive attitude to change and you are an excellent team player, we would like you to join our team to enable us to provide high-quality, patient-centred, holistic nursing care.

NMC Level 1 Registration (Adult Branch) & Enhanced DBS clearance required.

Car driver essential.

The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients’ homes or clinic settings.

Key Responsibilities

  • To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.
  • The role includes planning, implementing, and evaluating programmes of care, initiating changes to care, and escalation, if required, to the senior community nurse and the wider team.
  • To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families, and carers, referring to others for reassessment when required.
