The Adur Community Primary Care Network Team is looking to recruit a Registered Nurse
To join our friendly and supportive team based in Lancing, providing care to housebound patients across the Adur locality.
Whether you are newly qualified or an experienced nurse seeking a new challenge, Community Nursing offers a rewarding and varied role, including wound care, long-term condition management, palliative care, catheter care, safeguarding, and care home support.
Successful applicants will receive a structured induction programme and ongoing support to develop their skills and confidence within the community setting.

Duties
As a Band 5 Community Nurse within the Adur PCN Team, you will provide high-quality nursing care to housebound patients in their own homes and care home settings. Duties include:
- Assessment, planning, delivery and evaluation of patient care.
- Wound assessment and management.
- Catheter and continence care.
- Administration of medication, including insulin where competent.
- Supporting patients with long-term conditions and complex health needs.
- Delivering palliative and end-of-life care.
- Safeguarding adults and raising concerns where appropriate.
- Working collaboratively with GPs, therapists, care homes and the wider multidisciplinary team.
- Supporting hospital discharge and preventing unnecessary hospital admissions.
- Maintaining accurate clinical records and contributing to care planning.
