Job Overview
We are offering an exciting opportunity to join us as an NMC Registered Community Staff Nurse to work in our busy and supportive Farnborough Community Nursing Team.
This is a part-time position.
Many people would be unable to live at home without the support they receive from the community nursing teams and community services are receiving referrals for patients with more complex needs than ever before, with increasingly specialist treatment and care being provided by highly skilled staff.
Our Community Nursing teams assess, plan, and deliver nursing care to patients within a home/clinic setting, in line with evidence-based practice and by joining our team you will be able to experience the difference this makes to families within the community.
Due to the nature of community working, ability to travel across the patch to provide care to patients is essential.
This post is also suitable for newly qualified nurses.
Main duties of the job
As part of your role as a Community Nurse you will demonstrate commitment to providing high standards of holistic care to our patients.
You will be competent, or willing to gain competency in skills such as (but not an exhaustive list) long term condition management, line care, complex wound care, and bladder & bowel management.
Working in collaboration with the multi-disciplinary team and colleagues you will be expected to:
- Promote independence and support patient-centered care planning.
- Provide care directly to patients on the caseload with a range of nursing needs.
- Work autonomously, in line with your scope of practice, competence, and policies.
- Handover patient information to the wider team, document patient information effectively.
- Escalation of patients' needs as appropriate.
You will receive support into the role through induction, training, and beyond. There will be opportunities for personal development and career progression if desired.
Car driver is essential with access to a vehicle for work. Mileage is claimable via the trust expenses system.
