Role Overview
As a Support Worker, you will play a vital role within a multidisciplinary team (MDT) dedicated to helping individuals regain independence and achieve their rehabilitation goals.
Key Responsibilities
- Provide therapy-based support in community environments
- Empower clients to improve their functional abilities and quality of life
- Work alongside the community nursing team to provide treatment for patients within their own homes
- Deliver tailored therapy interventions under the guidance of qualified health care professionals
- Support patients in developing and practicing skills needed for daily living and independence
- Assist patients in setting realistic goals and motivate them to work towards achieving these objectives
- Monitor and record patients’ progress using programmes such as EMIS, and provide feedback to the MDT and lead health care professional
- Promote a safe and supportive environment, encouraging patients’ confidence and participation
- Collaborate with other team members, including physiotherapists, occupational therapists, and nurses, to ensure holistic care
- Encourage and facilitate community engagement and participation for patients

Qualifications and Experience
- Compassionate and patient with a strong desire to support others
- Good communication and interpersonal skills
- Ability to work independently and as part of the wider MDT team
- Basic understanding of therapy principles, rehabilitation techniques, and nursing skills and techniques (training provided if needed)
- Relevant experience in community settings is desirable

Working Hours
We have full and part-time hours available and can support flexible working patterns - currently this service runs from 8am - 8pm - 7 days a week.
