Role Description
The Community Support Worker is a full-time, on-site role based in Sutton In Ashfield. The role involves providing practical and emotional support to individuals in the community and in residential care home settings, following personalized care plans. Day-to-day tasks may include:

The Community Support Worker documents observations, reports any changes in needs or risks, and collaborates with colleagues and external professionals to coordinate care. The role also requires maintaining a safe environment, respecting confidentiality, and promoting independence and choice for each person supported.
Qualifications
- Strong Interpersonal Skills and Communication abilities to build trust and interact effectively with individuals, families, and colleagues.
- Experience or demonstrated interest in Community Support and Community Health to assist people in maintaining wellbeing and accessing local resources.
- Knowledge of Case Management principles to help coordinate services, follow care plans, and record accurate information.
- Basic understanding of safeguarding, health and safety, and person-centered care approaches.
- Ability to work reliably on-site in Sutton In Ashfield, including flexible hours where required.
- Previous experience in social care, support work, or related settings is beneficial but not always essential.
- Relevant vocational qualifications (e.g., health and social care certificate or diploma) are an advantage.
