We are looking for 6 Community Wellbeing Workers to join us as part of an exciting project to support people in their local community.
Community Wellbeing Workers (2 x 8 hours) – Portsea and Somerstown
Community Wellbeing Workers (2 x 8 hours) – Buckland and Landport
Community Wellbeing Workers (2 x 8 hours) – Paulsgrove and Wymering

The Wellbeing Service supports individuals living in Portsmouth to modify behaviours, improve their health and life expectancy as well as reduce health inequalities. The service provides support to Portsmouth residents in a range of venues including community centres, GP practices and hospital settings. In addition, the service provides telephone/virtual support. Some of the work undertaken will be targeting Portsmouth’s most deprived communities who unfairly suffer the greatest health inequalities.

As a Community Wellbeing Worker, you will build trusted relationships with residents and provide personalised support to improve their physical, mental and social wellbeing. Working with individuals and groups across a range of settings, including GP surgeries, community venues and residents’ homes, you will assess needs, develop tailored wellbeing plans and support positive behaviour change through areas such as physical activity, smoking cessation, weight management and reducing harmful drinking. You will also help people access support for wider issues that impact health and wellbeing, including housing, debt, isolation and domestic abuse. Working as part of a collaborative team and alongside health and care partners across the city, you will maintain accurate records, support referral and triage processes, contribute to community wellbeing events and play a key role in improving the health of Portsmouth’s residents. Who Is The Person Summary
We’re looking for a motivated and customer-focused individual with experience supporting people in one-to-one or group settings. You will have excellent communication and interpersonal skills, the ability to work effectively as part of a multi-disciplinary team, and be confident managing your own workload while working autonomously when required. An understanding of the wider determinants of health, local communities and the challenges people face in maintaining their wellbeing is important, along with strong organisational, IT, numeracy and literacy skills. You will be committed to safeguarding, equality, continuous professional development and delivering excellent support to service users. Experience of behaviour change approaches, healthy lifestyle support, partnership working, and working with community or voluntary sector organisations would be advantageous. A driving licence, or the ability to work across Portsmouth (PO1–PO6), is essential.
