Job Opportunity: Complex Care Nurse
Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare or working on the Staff Bank within these organisations. Nursing and AHP vacancies are also open to Student Nurses and Newly Qualified Nurses/AHPs.

About the Role
Are you passionate about making a real difference to people living with frailty and complex long-term conditions? Join our Complex Care Nursing team and help enable people to live well at home and reduces hospital admissions.
Working across a designated Primary Care Network, you will support a caseload alongside experienced Band 6 and Band 7 Complex Care Nurses. You'll undertake routine assessments, patient reviews and monitoring, supporting people with conditions such as Heart Failure, COPD and diabetes.
Key Responsibilities
- Phlebotomy
- Blood pressure and weight monitoring
- Continuous Glucose Monitoring (CGM) set-up and review
- Foot and skin assessments
- Administering medications including insulin
- Providing education to promote self-management and independence
