Working Hours: 17.5 Hours Per Week

Contract Type: Permanent, Part-Time

Working Style: Hybrid - Employees split their time between working remotely (e.g., from home) and working on-site (e.g., in an office).

Additional Payments: 6.7% Flexibility Payment

The Hospital Discharge to Assess service plays a key role in supporting timely, safe and person-centred discharge from hospitals for Manchester citizens. By working closely with health and social care partners, the service ensures that people are supported to leave hospital at the right time and receive the right support in the most appropriate setting.

This is an exciting opportunity to be part of a fast-paced, multi-agency service that has a direct impact on people’s outcomes, helping to promote independence and reduce delays in care. This post is offered as a 50% job share (17.5 hours per week). As such, it is essential that you can work in close partnership with your job share colleague, maintaining clear, consistent communication and a shared understanding of responsibilities. You will be expected to contribute to effective handovers, ensure accurate and timely recording of information, and take joint responsibility for managing workload, priorities and service demands. A collaborative and accountable approach is critical to maintaining continuity and ensuring a seamless service.

If you are successful, you will join a team of professionals who play a vital role in enabling safe and effective hospital discharge. As the first point of contact for referrals, you will ensure that information is processed accurately and efficiently, supporting people to move through the discharge pathways in a timely way.

Your role will be central to maintaining the flow of the service, working collaboratively with health and social care colleagues, and in close coordination with your job share partner to manage referrals, respond to queries and support timely and appropriate decision-making. This will require strong organisational skills, attention to detail, and the ability to adapt to changing demands while maintaining a consistent and reliable approach.

We are looking for an organised and proactive individual who thrives in a fast-paced environment and is able to manage competing demands effectively.

You will have strong communication skills and be confident working with a range of professionals across health and social care. You will be comfortable taking ownership of your workload, using your initiative to resolve queries, and ensuring referrals are managed accurately and efficiently.

You will also have excellent IT skills and be confident using multiple systems to support timely and accurate information sharing.

We would love to hear from you if you have experience of

  • Working in a busy administrative or customer service environment
  • Managing referrals, enquiries or coordinating processes
  • Working collaboratively with professionals across health, social care or partner agencies
  • Using digital systems to record and manage information accurately
  • Dealing with complex or sensitive enquiries

Essential requirements for the role

  • Excellent IT skills, including confidence using Microsoft Office and multiple systems to accurately record, manage and retrieve information
  • Strong interpersonal and communication skills, with the ability to engage effectively with a range of professionals and service users
  • Good planning and organisational skills, with the ability to manage competing priorities and meet deadlines
  • Ability to problem solve and make sound decisions within role boundaries, escalating appropriately where required
  • Ability to work effectively in a fast-paced, dynamic environment
  • Commitment to maintaining accurate records and handling confidential information appropriately
  • Experience of working in an administrative, customer service, or health/social care setting

Desirable requirements that would be useful in this role

  • Experience of working within health or social care services
  • Experience of referral management or coordination processes
  • Knowledge of hospital discharge processes or pathways
  • Experience of working with partner organisations or multi-agency teams
  • Familiarity with case management or recording systems used within health or social care
  • Working towards or holding a relevant qualification (e.g. administration, health and social care, customer service)
