Our ADHD Continuing Care Service supports patients once they have successfully completed their titration pathway, ensuring they continue to receive safe, effective, and responsive care throughout their treatment journey. We are seeking an experienced and motivated Team Leader to lead our operational team, ensuring patients remain supported regardless of whether Shared Care has been accepted or declined.
Role Overview
As Continuing Care Team Leader, you will oversee the day-to-day management of the Continuing Care team, ensuring patients receive a seamless experience following completion of titration.
You will lead a team responsible for supporting patients who have entered Continuing Care, including those with accepted Shared Care Agreements and those who remain under Medinet prescribing. You will ensure prescriptions, annual reviews, monitoring requirements, patient communications, and GP liaison are delivered safely, efficiently, and in line with clinical governance requirements.
You will act as the first point of escalation for operational issues, support service improvement initiatives, and work closely with clinicians, operational teams, and external stakeholders to ensure patients continue to receive high-quality care throughout their ongoing treatment.
Key Responsibilities
Team Leadership & Line Management
- Lead, motivate, and support a team of Continuing Care Coordinators and Administrators.
- Conduct regular 1:1 meetings, performance reviews, and coaching sessions.
- Foster a culture of accountability, compassion, and continuous improvement.
- Manage team rotas and ensure appropriate staffing levels to meet service demand.
- Support recruitment, onboarding, and training of new team members.

Continuing Care Operations
- Oversee the operational delivery of the Continuing Care pathway following patient discharge from titration.
- Ensure patients continue to receive appropriate support regardless of Shared Care Agreement outcome.
- Monitor prescription requests, annual reviews, physical health monitoring, and follow-up activities.
- Ensure patients are managed in accordance with clinical protocols, prescribing policies, and service standards.
- Support the management of patient recalls, medication reviews, and monitoring schedules.
- Ensure accurate maintenance of patient records across all clinical systems.
Patient & GP Communication
- Act as the primary escalation point for complex patient, GP, and pharmacy queries.
- Ensure patients receive clear, timely, and compassionate communication throughout Continuing Care.
- Support the coordination of Shared Care communications with GP practices.
- Liaise closely with clinicians regarding prescribing decisions, monitoring requirements, and patient concerns.
- Maintain excellent relationships with internal departments and external stakeholders.
Quality, Governance & Compliance
- Ensure compliance with GDPR, information governance, and clinical governance requirements.
- Monitor KPIs, service performance, and operational SLAs.
- Participate in audits, quality assurance reviews, and continuous improvement initiatives.
- Identify risks and implement actions to improve patient safety and operational performance.
- Support implementation of new systems, processes, and digital innovations.
Service Improvement
- Identify opportunities to improve patient experience and operational efficiency.
- Analyse performance data and trends to inform service improvements.
- Work collaboratively with Operational Managers and Clinical Leads to support service growth.
- Contribute towards new pathway development and future Continuing Care initiatives.
Essential
What You'll Need
- Previous experience leading or supervising a healthcare administration or patient support team.
- Strong understanding of patient pathways and operational service delivery.
- Excellent organisational skills with the ability to prioritise competing demands.
- Exceptional communication and stakeholder management skills.
- Experience using patient management systems (EMIS Web, Hero Health, or similar).
- Strong attention to detail and commitment to delivering high-quality patient care.
- Knowledge of information governance, confidentiality, and GDPR.

Desirable
- Experience within an ADHD, Mental Health, or Neurodevelopmental Service.
- Knowledge of Shared Care Agreements and prescribing pathways.
- Experience supporting long-term condition management or continuing care services.
- Experience using operational performance reporting and service KPIs.
