The team will establish contact with patients/carers from the time of initial referral or identification to provide specialist nursing advice and practical, psychosocial support.

• Communicate to patients in a way that empowers them to make informed choices about their health and healthcare.

We are an active part of the COPD biologics pathway and provide support and troubleshooting advice to patients regarding their treatment.

As a Respiratory Specialist Nurse within the Integrated COPD Team, you will be expected to:

• Have excellent communication skills.
• Promote the delivery of a high standard of patient-centred care.
• Work between organizational boundaries.
• Work across different organizations with the support of a Senior Sister.

You will analyse, monitor, and implement actions that ensure the service delivery is safe and effective in line with the Trust values. You will be expected to be a coach to our patients, their families, and hospital staff facilitating a better understanding of COPD.

The post holder will require a flexible approach to working patterns and will be required to work to a rota that includes weekends and bank holidays.

The post holder will need to be adaptable and flexible and work in varied environments, including the hospital, home visits, and on multiple sites.

Qualifications, Knowledge And Experience

Essential criteria

• Registered nurse on relevant part of the NMC register
• Degree qualification, 120 credits at L6 or possess an equivalent knowledge to be assessed through evidence of relevant specialist training, experience, or portfolio
• Significant post-registration experience/qualification/training, including experience and knowledge within the relevant specialty
• Completion of all Band 5 (preceptorship) competencies
• Higher Education Institute approved course in supporting learners in practice or evidence assessed to be of equivalent experience, demonstrating support to a range of learners in practice. Provide evidence of both supporting a range of learners in practice and supporting colleagues to promote effective learning in practice
• Extensive post-registration experience within the specialist area
• Knowledge of current issues within the acute health service
• Current UK driving licence with access to a vehicle
