Referral and Partnership Coordinator (0.6 WTE)

This is a two year fixed contract funded by Gloucestershire County Council as part of a pilot project. We are looking for someone who can work 22.5 hours per week over 4 days at £29,000 pro rata per annum.

Job Description

This role sits at the centre of our Crisis Resilience Fund (CRF) work across Stroud and Cotswold Districts.

You will act as the bridge between community hubs, referral partners, and individuals in crisis. Your job is to make sure no one falls through the gaps and no one has to tell their story twice. You will develop and strengthen relationships with appropriate support services and external partners to establish effective referral pathways, ensuring that people in need can access timely physical, practical, and holistic support.

This role will develop and implement a robust recording and monitoring system to capture referrals, outcomes, and the social impact of the service. This will provide the evidence base for evaluating the effectiveness of the service and support accurate reporting of compliance with the CRF contract requirements for Gloucestershire County Council. Utilising The Grace Network's economic power to address safety and physiological needs, you will facilitate access to essential household furniture, household goods, children's toys, meals, and other practical resources.

The Role

Partnerships & Referral Pathways

  • Build and maintain referral partnerships with community organisations, Gloucestershire County Council and County Council commissioned groups, local authorities, the NHS and voluntary sector.
  • Manage referral pathways including referrals and signposting to third party groups to ensure people can access the right support quickly.
  • Act as the key link between Grace Network hubs and external agencies.

Crisis Response & Service Delivery

  • Coordinate crisis support, including emergency vouchers, essential goods and practical assistance.
  • Help develop and implement a new system of applying for, managing and delivering crisis goods support.
  • Work with internal enterprises (Kids Stuff, Long Table, Bike Drop Gloucestershire House Clearances, The Home Remedy and The Great Plate and others) to provide timely access to goods and services for people in crisis.
  • Use local intelligence and community feedback to target support where it is most needed, including outside hub locations.

Monitoring, Learning & Development

  • Track referrals, outcomes and social impact, maintaining accurate records and reporting.
  • Use data and insights to monitor service effectiveness and inform continuous improvement.
  • Support the development of a scalable Community Response Framework (CRF) model across the county

As such, the postholder will be proactive, adaptable, and comfortable working in an environment where services and processes are still being developed. They will be able to respond dynamically to emerging needs, using initiative and sound judgement to shape and refine the service as it grows.

The successful candidate will be flexible in their approach, recognising that priorities may change as we gain a better understanding of the needs of the communities we serve.

They will be confident in developing new ways of working, establishing effective referral pathways with partner organisations, and building strong collaborative relationships to improve access to support. They will also be committed to continuous learning and service improvement, developing systems to record activity, measure outcomes, and capture the social impact of the service. This evidence will inform future service development and demonstrate compliance with the CRF contract requirements for Gloucestershire County Council.

Essential

  • Strong relational skills – able to build trust quickly across very different groups
  • Ability to coordinate multiple stakeholders
  • Comfortable working in ambiguity and building systems from scratch
  • Organised and detail-oriented without becoming bureaucratic
  • Able to handle sensitive information with care and integrity
  • Commitment to person-centred, trauma-informed approaches

Desirable

  • Experience working with people in crisis or vulnerable situations
  • Experience in social enterprise, community hubs, or VCSE sector
  • Understanding of local authority or NHS systems
  • Experience with referral systems or case management
  • Data tracking and impact reporting experience
  • Lived experience of navigating crisis support systems
