West Berkshire Council is seeking a dedicated and skilled Dementia Practitioner to join our team at Willows Edge Care Home, where you will play a vital role in ensuring the well-being and comfort of our residents.
About The Role
As the Dementia Practitioner, you will be responsible for:
- Delivering high-quality, person-centred care to residents with dementia
- Ensuring the safety and security of residents, responding to their needs promptly
- Working collaboratively with the night care team to provide effective care and support
- Monitoring and maintaining accurate records of residents' well-being and any significant changes
- Communicating with daytime colleagues to ensure smooth handovers and continuity of care
Fixed working pattern: Working every other Saturday and Sunday, or every Sunday.
About You
The ideal candidate will possess the following skills and attributes:
- A genuine passion for working with individuals with dementia and a commitment to enhancing their quality of life
- Previous experience in a similar role, providing care and support during the night
- Sound knowledge of dementia care principles and best practices
- Excellent communication and interpersonal skills, with the ability to build positive relationships with residents, families, and colleagues
- Ability to work effectively as part of a team, demonstrating collaboration and mutual support
- A proactive and solution-focused approach, with the ability to respond calmly to challenging situations

You will also be entitled to a generous annual leave entitlement starting at 27 days per annum (plus public holidays) Rising by an additional 5 days after 5 years’ service. (Pro rata for part time hours)
