Atrial fibrillation (AF) is the most common irregular heart rhythm, expected to become more frequent in the coming decade. Researchers are studying how differences in persistent AF mechanisms, especially beyond the pulmonary veins, relate to the heart's underlying condition. This research aims to develop personalized ablation strategies targeting specific conduction velocity changes and autonomic influences to improve ablation success rates and patient wellbeing. The study involves patients undergoing catheter ablation for persistent AF lasting less than 24 months without prior left atrial ablation. Participants are divided into groups based on the presence or absence of low voltage zones (LVZs) in the left atrium. Various studies within the trial will develop and test methods to identify sites of conduction slowing, assess autonomic modulation effects, and perform targeted ablation including ganglionated plexi (GP) site ablation and pulmonary vein isolation. Cardiac MRI will also be used to evaluate structural features related to AF mechanisms. Participants will undergo detailed mapping and pacing procedures during their ablation, including electrogram recordings and spectral analysis to identify key areas for ablation. Follow-up includes clinical visits and Holter monitoring over 12 months to assess freedom from AF or atrial tachycardia. The study will measure conduction velocity changes, ablation responses, autonomic modulation effects, and the impact of substrate-guided ablation on arrhythmia recurrence. Safety and procedural outcomes will be monitored throughout the study period.
