Job Introduction Diabetes Healthcare Assistant - Specialist Diabetes Service

An exciting opportunity has arisen for a Diabetes Healthcare Assistant to join our Specialist Diabetes Team based in Devizes.

Working under the supervision of Diabetes Specialist Nurses and alongside Specialist Dietitians, you will support patients with diabetes across community, hospital and domiciliary settings. Responsibilities include supporting diabetes clinics, assisting patients with diabetes technology, helping ensure completion of key diabetes care processes, and delivering structured diabetes education programmes.

Full training and accreditation support for delivering Xpert courses will be provided. We are looking for an enthusiastic individual with strong communication skills, confidence using IT and technology, and the ability to work independently whilst travelling across the locality as required.

This is a varied and rewarding role that requires a flexible approach to working. The successful candidate will support patients through a combination of clinic-based appointments, home visits and community-based activities across North West Wiltshire, helping patients to develop the skills and confidence needed to manage their diabetes effectively.

Base: Flexible base across Chippenham Community Hospital, Savernake Hospital, or Malmesbury Health Centre

Hours: 22.5 hours per week

Key Responsibilities

  • Support the delivery of specialist diabetes services across community, domiciliary and hospital settings.
  • Educate and train patients in the use of glucose monitoring systems, mobile applications and digital healthcare technology.
  • Manage a delegated caseload, prioritising referrals for diabetes education and technical support.
  • Deliver accredited Xpert structured education programmes for people living with Type 2 diabetes.
  • Undertake delegated clinical duties under the supervision of Diabetes Specialist Nurses.
  • Provide technical support for glucose monitoring devices, sensor insertion and data downloading.
  • Maintain accurate clinical documentation and patient records in accordance with organisational policies.
  • Participate in MDT working and contribute to high-quality, patient-centred diabetes care.

Skills, Experience and Attributes

This role requires a motivated individual who can combine excellent patient care with strong digital and communication skills. You will be comfortable supporting patients to use healthcare technology while working closely with specialist clinicians to achieve positive outcomes.

Essential Criteria

  • NVQ Level 3 or equivalent qualification.
  • GCSE English Grade A-C (or equivalent).
  • Ability and willingness to undertake additional training relevant to the role.
  • Previous Healthcare Assistant experience.
  • Experience working within a nursing or healthcare environment.
  • Experience using computers, mobile phones, apps and email confidently.
  • Strong understanding of IT systems and the ability to learn new technologies quickly.
  • Experience educating, coaching or teaching patients.
  • Experience working with individuals across different age groups.
  • Excellent verbal and written communication skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Professional, patient-centred and empathetic approach.

Desirable Criteria

  • Experience supporting diabetes services or long-term condition management.
  • Experience delivering presentations or education to groups.
  • Knowledge of diabetes technologies such as glucose monitoring systems.
  • Experience within community healthcare settings.
  • Awareness of structured patient education programmes.
