Job Summary
Are you ready to make a real difference in diabetes care? We’re looking for a passionate Specialist Diabetes Practitioner to join our forward-thinking team within the Northern Care Alliance. This is your chance to work autonomously, delivering a high-quality, evidence-based service that transforms patient outcomes and empowers people to manage their own diabetes.
In this role, you’ll assess, plan, and implement individualized care while acting as a trusted resource for patients, carers, and healthcare professionals. You’ll also play a key part in shaping the future of diabetes services by delivering specialist education and training to staff and students across all aspects of diabetes management.
This is a varied and rewarding position with cross-site cover—4 days at Royal Oldham Hospital and 1 day at Fairfield General Hospital—giving you the opportunity to broaden your experience and collaborate with a dynamic multidisciplinary team.

Role Summary
We are seeking a motivated Specialist Diabetes Practitioner to join our team within the Northern Care Alliance. In this autonomous role, you will deliver a high-quality, evidence-based service for people living with diabetes, working within Trust policy, national guidelines, and your professional scope of practice.

Your responsibilities include:
- Reducing inpatient bed occupancy by promoting excellent diabetes care and empowering patients to manage their condition.
- Assessing, planning, implementing, and evaluating individualized treatment plans, ensuring appropriate care pathways and referrals.
- Acting as a specialist resource, providing expert advice to patients, families, and healthcare professionals.
- Delivering education and training to staff and students on all aspects of diabetes management.
- Participating in research and clinical audits.
- Maintaining compliance with NMC registration.
- Taking accountability for your professional development through supervision and appraisal.
Training opportunities will support service development and progression, including working towards or completing NMP V300.
