Vacancy Introduction
Are you a diabetes specialist nurse looking for a new role in the community, then we have the role for you.
We are currently recruiting to two full-time band 6 diabetes specialist nurse roles. To be eligible for this role you must currently work or have worked previously as a diabetes specialist nurse. The role can however be offered as a band 5-6 development role for anyone with diabetes experience, but who has not worked as a diabetes specialist nurse before.
We have an exciting opportunity for a diabetes specialist nurse to join our multi-disciplinary team, made up of nurses, dietitians, GPsi's, diabetes facilitators and pharmacists. If successful you would be working across Leeds to provide face to face and telephone clinics, carrying out home visits and providing education groups for patients with type 2 diabetes.
This role would suit someone who is enthusiastic and committed to providing excellent diabetes care, and who wants to further develop their knowledge and skills in this speciality. We, as a team, are committed to staff wellbeing and development, and the successful candidate will be supported to completed NMP if not already done so as this is an essential requirement of the role.

The role requires excellent communication skills as you will undertake complex assessments of patients with diabetes, implement treatment plans and then review and assess, all while promoting self-management for the person with diabetes.
Working hours are predominately 8:30 - 4:30 Monday to Friday.
Car owner/driver is essential for you to undertake this role.
