Exciting Opportunity for a Community Diabetes Specialist Nurse
An exciting opportunity has arisen for a Community Diabetes Specialist Nurse to join the team at UHCW.
Role Overview
A Diabetes Specialist Nurse requires expertise in the management of diabetes in both hospital and community settings. The role is educational and advisory with the aim of providing specialist diabetes care to the person with diabetes, their carer, and training for healthcare professionals.
Key Responsibilities
- Work autonomously as well as within the multidisciplinary team to achieve targets set out within National guidelines.
- Manage and organize a specific caseload of patients and prioritize referrals.
- Accept referrals from other health disciplines and refer patients to health professionals independently.
- Assist patients to achieve individualized goals of diabetes management to maintain their physical wellbeing.
- Advise medical and nursing staff on the most appropriate treatment for their patient based on clinical assessment and clearly defined policies and protocols.
- Convert patients to insulin therapy on an inpatient and outpatient basis.
- Advise patients on adjustment of their oral hypoglycaemic agents or insulin treatment to optimize control of their diabetes.
- Select, carry out, and interpret the results of specific tests and take appropriate action.
