Job Title: Nurse Specialist
As the nurse specialist within the defined sphere of practice, the post holder is responsible for providing specialist knowledge and care to patients. They will act as the first point of contact for those needing to access the specialist service. The role involves undertaking specialist procedures, providing specialist advice, structured education, and ongoing support to the client group based on individual needs.
Responsibilities
- Provide specialist knowledge and care to patients.
- Act as the first point of contact for those accessing the specialist service.
- Undertake a range of duties to ensure effective delivery of care.
- Provide specialist procedures, advice, education, and support to the client group.
- Support the development and implementation of the integrated diabetes service across the Northern Lincolnshire and Goole Clinical Commissioning Groups (CCG) in collaboration with other community providers to improve the quality of diabetes care.
