About Us
This post is in our hospital discharge/LIFE team focusing on hospital discharge. This work is short term but fast paced and some weekend working is required on a rota basis. You will work with other professionals in an integrated service providing strengths-based assessments for reablement in people's own homes.
The Hospital Team facilitates discharges from Croydon University Hospital and all Out of borough hospitals.
We work within the Transfer of Care Hub Team (TOCH) consisting of multiple professionals working together to facilitate swift and suitable discharges destinations.
The post holder will be responsible for case managing and facilitating safe and effective discharges of patients from hospital to a community setting through a multidisciplinary approach. They will coordinate, oversee Discharge Navigators within their sub-team, complete assessments of individual needs and support the discharge plan. The post holder will be proactive in identifying concerns or issues that may delay effective discharges and work with the Transfer Of Care Hub Team (TOCH) to overcome and limit these.
The successful candidate will work closely with all ward teams ensuring appropriate referrals are seen and made to local authorities and preventing unnecessary, avoidable delays for patient discharge.
This role will extend to support and ensure that individuals who have been discharged feel supported and have received the support that was requested.
About the Role
- Liaising with the hospital and professionals to facilitate timely discharges.
- To undertake Strength/Asset Based/Assessments and or reviews, sign, post, respond to enquiries and to implement support packages where required to minimise the risks identified In line with the Care Act 2014 and Community Led Support.
- Completing Care Act Assessments to determine ongoing care needs.
- Providing information and advice.
- Work in partnership with health professionals in the Integrated Discharge Team.
- To triage safeguarding initial concerns raised, information gathering and participating in SAM discussions to make decisions as to whether concerns raised need a formal s. 42 or pass to care management.
- Work in partnership with service users, carers and family members.
- Ensure a high standard of case recording.
- Complete risk assessments and implement protection plans as required as part of crisis intervention role and responsibilities.
- Undertake Mental Capacity Assessments when required.
- Complete referrals to Advocacy services.
- Deputise in manager's absence.
