Ealing Hospital Discharge Hub Opportunity
The Ealing Hospital Discharge Hub has an exciting opportunity for an enthusiastic and motivated individual with care experience and experience within hospital discharge pathways, discharge planning, and working within a multidisciplinary team.
Role Overview
Your main role will be to support P1 discharges from Ealing Hospital into the community in a safe and timely manner. You will hold an active daily caseload of patients and ensure they are discharged within the agreed timeframe. Additionally, you will complete pre-checklists for P1 patients being discharged.
Key Responsibilities
- Work in partnership with colleagues in the acute sector, social services, Ealing Community Partners, ICB, and other community health services to ensure that appropriate patients from the acute setting are assessed and transferred safely to appropriate community-based services in a timely and seamless manner.
- Support P1 discharges from Ealing Hospital into the community in a safe and timely manner, hold an active daily caseload of patients, and ensure they are discharged within the agreed timeframe. Complete pre-checklists for P1 patients being discharged.
- Work as a core member of the acute-based Ealing Hospital Discharge Hub team to build and deliver the shared vision for a hospital discharge service.
- Be responsible for optimizing hospital bed utilization and reducing the length of stay by ensuring the timely, safe transfer/discharge of pathway 1 patients to the most appropriate and agreed environment.
- Act as a proactive community health service advocate, on behalf of West London NHS Trust, through extensive knowledge of relevant community-based pathways within the area of operation and ability to support effective discharge planning.
- Work in close partnership with hospital colleagues and external agencies such as Social Services, ICB, Housing Associations, GPs, and voluntary organizations. This will involve attending meetings outside the Trust and sharing Care Management Plans with the ASC and other community providers.
