Community Case Manager/District Nurse
This role is part time and is 30 hours a week.
Role Overview
The role is providing nursing interventions to patients in their home environment. If you are interested in joining our friendly team, we would love to hear from you. You must be a car driver with access to a car you can use for work.
In this role you will become a competent Community Nurse. Our skills include IV's, catheter care, palliative care and end of life support, leg ulcer management, wound care, medication management, complex care management, prevention of admission work and MDT working.
Our days are varied, with teamwork and compassion being at the heart of everything we do. Amersham ACHT aim to provide the highest standard of care for our patients. We will support you through your induction and gaining your skill competencies.
Responsibilities
- To provide interventions tailored to meet the needs of the individual
- To contribute to processes for effectively caseload management for patients in community setting who are very high risk of acute deterioration and unplanned admission to hospital
- To assess the physical and psycho–social needs of the defined case load instigating therapeutic treatments and care delivery based on best available evidence
- To improve health outcomes using specialist nursing skills and knowledge
- To undertake referral management to develop and establish case management programmes for patients referred to the Adult Community Healthcare Teams
- To manage a caseload of patients with long term conditions who would benefit from case management

Requirements
Ideally you should have a wide range of clinical expertise within the NHS as well as experience working in a multidisciplinary team environment. The post holder must be able to demonstrate their professional skills and competence in medicines management, care and treatment delivery. You must be innovative in your approach and willing to work with other members of the community team and ward staff to ensure a high-quality service delivery. You must be willing to develop your own knowledge and skills. We provide a 7am to 9pm service 7 days a week.
