Job Details
- Hours: Full time 37.5 hours a week
Team Leadership
- Community Nursing Matron
- District Nurse Team Lead: Qualified as Specialist Practitioner Qualification (SPQ) for District Nursing
- Advanced Case Manager: Extensive skills managing teams
About the Team
The ACHT reflects an integrated approach designed in response to the changing needs of the local population, with a special emphasis on proactive care for patients in the community setting. This includes:
- Caseload management
- Prevention of admission
- End of life care
- Facilitating timely discharge from hospital utilizing a coordinated approach to care in partnership with key care partners
Role Responsibilities
- Provide interventions tailored to meet the needs of the individual
- Contribute to processes for effectively managing caseloads for patients in the community setting who are at high risk of acute deterioration and unplanned admission to hospital
- Assess the physical and psycho-social needs of the defined caseload, instigating therapeutic treatments and care delivery based on best available evidence
- Improve health outcomes using specialist nursing skills and knowledge
- Undertake referral management to develop and establish case management programmes for patients referred to the Adult Community Healthcare Teams
- Manage a caseload of patients with long term conditions who would benefit from case management
