Centralised Triage Lead
Are you an experienced community nurse leader with a passion for service transformation, clinical excellence and patient flow?
We are looking to hire a Centralised Triage Lead to develop and lead the delivery of a centralised triage across District Nursing services.
About the Role
This is a pivotal role that will help shape how patients access care, ensuring referrals are managed safely, efficiently and consistently while supporting integrated working across community services.
As the Centralised Triage Lead, you will provide both operational and clinical leadership, ensuring that referrals are appropriately prioritised and allocated according to patient need, service standards and clinical risk.
Key Responsibilities
- Provide clinical and operational leadership for the Centralised Triage Service.
- Develop and implement triage pathways, standard operating procedures and referral management processes.
- Ensure referrals are clinically assessed and prioritised in line with service criteria and risk management frameworks.
- Monitor demand, capacity and workforce to maximise service effectiveness.
- Play an active role in service redesign and transformation projects relating to access, assessment and referral management.
- Work closely with District Nursing Teams, senior leadership and partner organisations.
- Use data and performance information to identify trends, risks and opportunities for improvement.
- Support governance, quality assurance and patient safety initiatives.
- Lead and develop staff through supervision, appraisal and professional development.
- Act as a clinical expert resource for complex referrals, case discussions and escalation decisions.
