Join our CQC-rated Outstanding Trust as a District Nursing Sister within the Windsor and Maidenhead Community Nursing Team and play a vital role in delivering exceptional care to patients in their own homes.
This is an exciting opportunity for an experienced and motivated Nurse to combine clinical expertise with leadership responsibilities. Working under the delegation of the Team Leader, you will manage a clinical caseload while providing day-to-day leadership and management to the District Nursing team. You will ensure high standards of patient care, promote evidence-based practice, and support the delivery of safe, effective, and responsive community services.
About the Role
As part of our supportive Community Nursing Team, you will assess, plan, implement, and evaluate care for patients with a wide range of health needs. You will be passionate about delivering person-centred care, enabling patients to remain independent and receive treatment in the comfort of their own homes.
Key Responsibilities
- Assess, plan, implement, and evaluate care for patients with a wide range of health needs.
- Provide day-to-day leadership and management to the District Nursing team.
- Ensure high standards of patient care, promote evidence-based practice, and support the delivery of safe, effective, and responsive community services.
- Support staff to complete holistic assessment and reassessment of all health and nursing needs.
- Manage a complex and diverse caseload of patients.
- Provide clinical support, leadership, and guidance to all staff to ensure that evidence-based care and clinical practice are maintained in Community Nursing Teams.

Qualifications and Skills
- Qualified District Nurse with a valid NMC PIN.
- Strong leadership, clinical decision-making, and organisational skills.
- Full UK driving licence and access to a vehicle for work purposes.
