An exciting opportunity has arisen for an enthusiastic and motivated Nurse, Occupational Therapist, or Social Worker—whether you're an established Band 6 practitioner or ready to take the next step in your career—to join our highly regarded Early Intervention in Psychosis (EIP) Team.

As a Care Coordinator, you'll become part of a vibrant multidisciplinary team of experienced professionals who work collaboratively with service users and their families using a bio-psycho-social, spiritual, and recovery-focused approach. You'll be encouraged to bring your ideas, expertise, and passion while being supported every step of the way.
Working in partnership with service users, carers, and other partner agencies, assess, plan, and provide packages of care for individuals and their families/carers experiencing first episode psychosis. To fulfil all requirements of CPA Care Co-ordination. Provide supervision and support to junior team members and colleagues.

Travel Requirements
The role involves regular travel throughout Bradford, Airedale, and Craven to meet the needs of the service and our communities. Therefore, applicants must be able to travel across the district to deliver high-quality care and support where it is needed most.
