We are recruiting for an Embedded Housing Worker, to work within our embedded team across Oxfordshire Mental Health and Oxford University Hospitals.
About The Service
Our Embedded Housing Service works alongside Oxfordshire hospitals to help patients overcome housing challenges that may impact their discharge. By connecting people to the right accommodation, support, and services, we help prevent homelessness and promote positive recovery outcomes.
The impact you will have
As an Embedded Housing Worker, you will support patients whose housing circumstances are affecting their ability to leave hospital safely. Working closely with hospital teams, housing providers, and support services, you will help people access suitable accommodation and the support they need to move forward.
Key responsibilities include:
- Identifying housing barriers and supporting safe hospital discharge.
- Working with housing providers, local authorities, and support agencies to secure positive outcomes.
- Providing housing-related information, guidance, and referrals.
- Supporting access to accommodation, homelessness services, and Stepdown provision where appropriate.
- Maintaining accurate records and monitoring outcomes.

Contract details:
- Contract: Permanent, subject to funding
- Salary: £28,541 - £32,554 per annum, dependent on experience
- Hours: 37.5 hours per week, Monday - Friday.
- Closing date: Thursday 8th October
- Introduction calls: Week commencing 12th October

About You
We're looking for someone who is passionate about helping people overcome housing challenges and achieve positive outcomes. You may have experience working in homelessness, housing, mental health, or a related support setting, and you'll be committed to providing person-centred support that makes a real difference.
You'll bring:
- Experience of working within homelessness, housing, mental health, or a similar support environment.
- An understanding of the Homelessness Reduction Act and the challenges faced by people experiencing housing insecurity.
- The ability to build positive relationships and work collaboratively with a range of professionals and partner organisations.
- Experience of maintaining accurate records and using IT systems, including Microsoft Office applications.
- The ability to research local services and community resources to help people access the support they need.
- Knowledge of housing options, welfare benefits, and housing application processes would be beneficial, although full training will be provided.
- Full driving licence and access to a vehicle.
This role is based within a busy hospital environment, so you'll be someone who can remain organised, work effectively under pressure, and confidently engage with a range of professionals.
