Job Introduction
Are you passionate about helping people regain their independence and achieve the things that matter to them?
We’re looking for an Enabler to join our Intermediate Care and Reablement Service, working as part of a multidisciplinary team to support people within the community.
Rather than simply doing things for people, reablement is about working with them. You’ll encourage, motivate and support service users to rebuild their confidence and abilities, following individual care and reablement plans designed to help them become as independent as possible.
No two visits will be exactly the same. You may be helping someone safely mobilize around their home, supporting them with washing and dressing, encouraging them with everyday activities, assisting with their dietary needs or supporting them to work towards rehabilitation goals set by the therapy team.
You’ll build positive relationships with service users and their families while working closely with therapists, clinicians, social care colleagues and other members of the multidisciplinary team.
This is a fantastic opportunity for someone with experience in health or social care who wants to play a hands-on role in helping people achieve meaningful outcomes and maintain their independence.
Please note: a full UK driving license and access to a car for work purposes are essential for this role.

As an Enabler within our Intermediate Care and Reablement Service, you’ll be part of a dedicated team supporting people to regain their independence and confidence following illness, injury or a change in their circumstances.
You’ll work directly with service users in the community, helping them achieve their individual reablement goals and supporting them to remain as independent as possible.

Main Responsibilities
As an Enabler, you will:
- Support service users to achieve their individual care and reablement goals
- Encourage and motivate people to regain and maintain their independence
- Support service users with personal care, including washing and dressing where required
- Support and encourage safe mobility, transfers and activities of daily living
- Deliver agreed therapy and rehabilitation activities in line with individual plans
- Support service users with their dietary needs
- Prompt and administer medication in line with individual needs, training and relevant procedures
- Observe service users and recognize changes or deterioration, escalating concerns promptly to the appropriate clinician or supervisor
- Build professional and supportive relationships with service users, their families and carers
- Work closely with therapists and the wider multidisciplinary team to help deliver joined-up care
- Maintain accurate and timely records of care, interventions, progress and outcomes
- Communicate effectively with colleagues regarding progress, concerns and changes to a service user’s needs
- Work independently in the community while knowing when to seek advice or additional support
- Travel efficiently between visits to ensure service users receive timely and effective support
- Maintain the privacy, dignity and choices of service users at all times
- Contribute to team meetings, training, service improvement and quality initiatives

The Ideal Candidate
You’ll be someone who takes genuine satisfaction from helping others achieve greater independence.
You’ll be compassionate, encouraging and patient, with the ability to build trust with people who may be recovering from illness or injury, adapting to a change in their abilities or working towards becoming more independent at home.
You will have:
- NVQ Level 2 in Health & Social Care or an equivalent qualification
- Experience of working within a multidisciplinary environment
- Experience within community care, rehabilitation, intermediate care or an integrated care service
- An interest in rehabilitation and reablement and helping people to achieve greater independence
- Good verbal and written communication skills
- Confidence using IT systems to maintain accurate care records
- The ability to work independently while recognizing when support or clinical advice is required
- The ability to organize and prioritize your workload
- A compassionate, patient-focused and professional approach
- A willingness to learn, develop and complete relevant training and competencies
