JOB TITLE: Exercise Specialist
DEPARTMENT: Chelsea FC Foundation-Community
LOCATION:
Chelsea Football Club, Stamford Bridge, Fulham Rd, London, SW6 1HS. (this is a delivery focused role and the successful candidate will work from various community locations)

Permanent, Full Time or Part Time positions available
JOB FUNCTION:
To lead Chelsea FC Foundation’s delivery across existing and new Population Health Programmes. Specifically developing and delivering specialist group exercise for long term health conditions. The role will work closely with partner agencies and local NHS Trust to recruit patients and community members on to programmes, identify suitable progression routes and develop opportunities that support healthcare inequalities within our local community.

MAIN RESPONSIBILITIES:
- To work closely with the Health and Wellbeing team as the lead deliverer for new and existing strands of Bridging the Blues Programmes, in line with the requirements set out by the funding providers.
- To support recruitment and referrals of participants from NHS pathways onto CFCF programmes.
- Deliver a range of exercise classes for specific health needs, including but not limited to: Stroke, Respiratory, Cardiovascular Disease and Cancer and providing support at suitable times for the referral either face to face or virtually.
- To work alongside the Health and Wellbeing team to develop new opportunities to support healthcare inequalities in our local community.
- Maintain and develop effective partnership working with stakeholders including relevant external agencies and organisations.
- Prepare delivery programmes and sessions plans that support the project outcomes. Ensuring participation forms, recording sheets and registers are completed accurately and entered in CFCF’s monitoring and evaluation systems.
- Ensure all CFCF health & safety and safeguarding procedures are implemented prior to any activity starting, including quality assuring casual staff delivery and ensuring all participants have undertaken the appropriate Physical Activity Readiness Questionnaire and exercise referral screening calls.
MEASURES OF PERFORMANCE:
- To be a positive role model, creating a safe, positive, and fun environment in which to motivate and encourage the community to live a healthy and active lifestyle.
- To build relationships where possible with local authorities, NHS Trusts & Charities, key community partners, youth/adult services and health & wellbeing charities.
- To achieve all KPI’s and Outcomes as agreed with the funding provider and Population Health Senior Officer.
- Establish, maintain and develop effective partnership working with stakeholders including relevant external agencies and organisations.
- To support the monitoring and evaluation for all Population Health projects in accordance with the needs of each funder, including ensuring all data is captured at session and input on Salesforce.
- Be responsible for the safeguarding of participants across all Population Health programmes, liaising and reporting into the Safeguarding Lead for Health and Wellbeing and associated partner Safeguarding Team’s to ensure a clear line of reporting to CFC Safeguarding Hub.
- Ensure success stories are captured through working with Chelsea FC Foundation/Chelsea FC media staff.
- Complete and submit project reports at 6-month intervals, or as and when required internally or in accordance with the needs of each funder.

SKILLS/KNOWLEDGE & EXPERIENCE
Qualifications:
Essential
- Relevant professional and/or academic qualifications or a minimum of 2 years’ experience in the industry sector.
- A minimum level 4 Fitness Qualification (or willing to work towards)
- FA Safeguarding qualification or equivalent.
- FA Emergency First Aid Certificate or equivalent.
- Physical Activity and/or Sports Coaching Qualification
- Ability to travel to activity & meetings across South-West London.
Desirable
- Level 3 Exercise referral qualification
- Nutrition qualification
- Health Promotion/Public Health/Health and Social Care or equivalent qualifications.
Experience:
Essential
- Knowledge and experience of delivering group exercise sessions to participants with complex medical conditions.
- Experience of implementing safeguarding and health and safety procedures within a physical activity setting to groups with comorbidities.
- A strong knowledge of and experience in working in a Charity, and/or Health and Social Care industry.
- Have a good understanding of NHS, Health and Social Care services and charities across London, be able to signpost participants accordingly, and build partnerships.
- Knowledge and experience of developing workshops and activity on Health and Wellbeing
- Previous experience in working within a community setting.

- Knowledge of the Chelsea FC Foundation.
- Knowledge of local demographic and social needs.
- Experience of coordinating delivery staff & volunteers.
- Knowledge of Community Club Organisations and Football Charity Programmes.
