Job Summary

We are seeking a dedicated and highly organised Care Coordinator to join our team, with strong experience in day-to-day domiciliary (home care) coordination. Applicants must have previous experience working within day domiciliary care services, including managing rotas, coordinating care visits, and supporting care staff in a community/home care setting.

The successful candidate will be responsible for managing client care plans, coordinating services, and maintaining effective communication between clients, families, and healthcare professionals, ensuring the smooth and consistent delivery of high-quality care.

The office is in Brentford, but the working area will be between Hammersmith and Fulham Borough, Kensington and Chelsea, as you need to visit the residents and their families, for spot checks and other important checks that need to be done.

Responsibilities

  • Coordinate and oversee individual care plans, ensuring all services are delivered according to client needs and organisational standards
  • Conduct home visits, spot checks, and other required quality assurance checks
  • Act as the primary point of contact for clients, families, and healthcare professionals, facilitating clear and timely communication
  • Schedule appointments, visits, and services while maintaining accurate records of all interactions and updates
  • Monitor client progress and report any concerns or changes to relevant team members promptly
  • Maintain organised documentation in compliance with data protection regulations and organisational policies
  • Support administrative tasks such as filing, data entry, and managing correspondence to ensure smooth office operations
  • Collaborate with multidisciplinary teams to ensure comprehensive care delivery and continuous improvement of services

Requirements

  • To have at least 1 year experience
  • Have NVQ level 3 in Health and Social Care

Proven experience in day domiciliary care coordination (essential)

  • Strong office and administrative experience
  • Excellent organisational and communication skills
  • Ability to work independently and manage a busy workload
  • Comfortable conducting visits within the community
  • Attention to detail to ensure accuracy in documentation and scheduling
  • Proficiency in standard office software programmes (e.g., MS Office Suite)
  • A genuine interest in providing quality care and support services

This role is strictly for candidates with prior day domiciliary care coordination experience. Applicants without this background will not be considered.
