Salary: £28,000 – £33,000 per annum (dependent on experience)

Job Description

Vivid Community Care is currently seeking an experienced Domiciliary Care Assessor to join our growing team and support the delivery of safe, effective, and compliant care services in the London Borough of Hammersmith & Fulham.

The Domiciliary Care Assessor will be responsible for conducting service user assessments, developing and reviewing care plans, carrying out risk assessments, and monitoring care quality to ensure compliance with CQC regulations and company policies.

The successful candidate will work closely with service users, families, healthcare professionals, and care staff to ensure outstanding standards of care are maintained at all times.

Key Responsibilities

  • Conduct initial assessments for prospective service users in their homes.
  • Complete comprehensive care plans and risk assessments tailored to individual needs.
  • Review and update care plans regularly to reflect changing care requirements.
  • Carry out spot checks, supervisions, and competency assessments for care staff.
  • Monitor and maintain compliance with CQC standards, safeguarding procedures, and health & safety regulations.
  • Support the implementation of person-centred care practices.
  • Liaise professionally with service users, relatives, social workers, healthcare professionals, and external agencies.
  • Maintain accurate, confidential, and up-to-date documentation and records.
  • Identify areas for service improvement and report concerns appropriately.
  • Support staff induction, training, and ongoing development where required.
  • Participate in audits, quality assurance checks, and compliance review

What We’re Looking For

  • Previous experience within domiciliary care or health and social care settings.
  • Confident in using care management and scheduling software, with strong knowledge of care planning, scheduling systems, and CQC requirements
  • Excellent communication and time-management skills
  • Ability to multitask and work under pressure in a fast-paced environment
  • NVQ Level 3 in Health and Social Care (or working towards)
  • Strong team player with a positive and flexible approach.
  • Committed to providing person-centred, high-quality care

Additional Requirements

  • Must undergo an enhanced DBS (Disclosure and Barring Service) check
  • Must provide three years of references
  • Must successfully complete a competency-based interview
