We’re looking for an experienced Care Coordinator to join our team across Bristol & the South West.

💼 Full-time | Permanent
💷 £29,000–£31,000 per year depending on experience
💙 What you’ll be doing
- Coordinating care rotas and staffing
- Supporting, supervising and developing care staff
- Managing referrals and personalised care plans
- Matching the right carers with the right clients
- Working alongside healthcare professionals and local authorities
- Arranging care reviews and risk assessments
- Supporting CQC compliance and high standards of care
- Taking part in a shared on-call rota

- 2+ years’ experience in domiciliary care coordination
- Excellent communication and leadership skills
- A genuine passion for delivering exceptional care
- A full clean UK driving licence and your own car
