Role Overview
Working within Oldham Council’s Older People’s Mental Health Team. You will be working within a specialist multi-disciplinary team supporting people with a severe and enduring mental health condition. To have responsibility for a caseload and fulfilling statutory duties such as undertaking assessments, developing care & support plans, and completing reviews, including Section 117 reviews. To continue to develop professionally, and be accountable for the delivery of, and promote, excellent social work practice.
We are a friendly team who prioritise team cohesion and wellbeing, having monthly supervisions, team meetings and peer reflective sessions. We also support CPD, providing protected time, with a commitment to support people through AMHP, BIA, Practice Educator, and Leadership Programmes.

Key Responsibilities
- To fulfil statutory duties under legislation including the Care Act, Mental Health Act, Mental Capacity Act, DOLS and Making Safeguarding Personal.
- Complete assessments, support plans, risk assessment and risk management plans, safeguarding enquiries and commission packages of care and care home placements.
- Work in a strengths based way to empower and promote the person’s independence
- Safeguard vulnerable individuals by proactively identifying and responding to safeguarding concerns, crisis situations, and legal interventions, ensuring compliance with relevant procedures and legislation.
- Maintain accurate, high-quality records on case management systems (e.g., MOSAIC), produce professional reports, and protect the confidentiality of personal information.
- Work collaboratively with multi-agency partners, advocate for service users, and contribute to service improvement through supervision, performance reflection, and ongoing professional development.
Key Requirements
To succeed in this role, you will need as a minimum:
- BA Hons degree in Social Work, have completed a Social Work Apprenticeship or equivalent
- Proven ability to implement, monitor, evaluate, and update Care & Support Plans using person-centred planning principles and relevant procedures.
- Experience in conducting accurate and sensitive assessments of Service Users, Carers, and Families to develop tailored Care and Support Plans, with evidence of strong analytical and report-writing skills. Experience in collating information and producing reports suitable for formal settings.
