We have an exciting opportunity for a Band 6 Nurse/Social Worker/OT to join us in delivering community mental health care to the Flexible Assertive Community Treatment (FACT) Team within East & South East Hertfordshire.
The area is well served with transport links to historic Hertfordshire and its glorious countryside as well as into London and beyond. We are looking for a Nurse/Social Worker/OT who is passionate about delivering care to adults with mental health issues.
As a Social Worker in an Adult Community Mental Health Service (ACMHS) you will:
- Provide clinical expertise and leadership
- Participate in the referral and allocation process
- Safeguard vulnerable children and adults
- Contribute to case discussions and service related meetings
- Manage a caseload
- Provide direct support and advice to Service Users (Adults of all ages) and Carers, within the recovery framework, embracing the individual right to choice and control
