Job Details
This role is in support of Cencora's patient and provider support and pharma commercialization services in Canada marketed through our Innomar Strategies business.

Responsibilities
- Delivers routine to moderately complex clinical services activities that support safe, accurate, and compliant care coordination, documentation, and case progression within established guidelines.
- Coordinates with clinical, operational, reimbursement, and logistics partners to resolve standard exceptions and maintain continuity across connected service and care workflows.
- Analyzes patient, provider, and clinical information to determine appropriate next steps, distinguish operational issues from clinical-risk signals, and escalate concerns within defined protocols.
- Resolves common workflow and documentation exceptions by applying approved decision rules, service standards, and clinical process requirements to support timely and reliable outcomes.
- Maintains accurate, complete, and traceable records that support quality controls, regulatory expectations, and effective handoffs across the service line.
- Improves templates, job aids, and routing practices based on recurring patterns to reduce rework, strengthen consistency, and enable low-risk process enhancements within team control.
- Facilitates day-to-day coordination and informal onboarding support through examples, question resolution, and clear communication of approved documentation and workflow expectations.

Education
- Bachelor's degree in nursing, clinical practice, health sciences, public health, healthcare administration, or a related field, or equivalent experience required.
- Professional license or registration in nursing in the applicable jurisdiction of practice required.
- Certification in clinical services, care coordination, patient safety, or equivalent preferred.
- Certification in case management, clinical documentation improvement, healthcare quality, or equivalent preferred.
Work Experience
- 4+ years of experience in nursing, clinical services, care coordination, patient support, clinical documentation, or a related field required.
Skills & Knowledge
- Flexibility in order to provide services for pre-scheduled and occasional last minute patient appointment requests
- Effective organizational skills; attention to detail
- Ability to communicate effectively both orally and in writing
- Strong written and verbal communication skills
- Able to work in both a team and independent setting
- Able to travel within a geographical boundary
