The Forensic Outreach and Liaison Service (FOLS)

The Forensic Outreach and Liaison Service (FOLS) is a community-based service in Sussex providing holistic support to individuals with a mental health issue that relates to their offending/risk behaviour.

About the Role

We are looking for a full-time, substantive Band 7 mental health specialist to work with our out-of-area patients, to help plan their discharge pathway from hospital into the community.

The role entails carrying out specialist assessments and identifying pathways of care for each patient, whilst working closely with the multi-disciplinary team (MDT). The role encompasses clinical and also developmental aspects, including taking a lead in service evaluation and quality improvement.

Key Responsibilities:

  • Carry out specialist assessments and identify pathways of care for patients.
  • Work closely with the multi-disciplinary team (MDT).
  • Take a lead in service evaluation and quality improvement.
  • Make recommendations to help individuals move towards discharge.
  • Monitor and assess the progress of individuals on a regular basis.
  • Evaluate the effectiveness of interventions.

Requirements:

  • A professional qualification is a necessity (e.g., RMN, OT, Social Worker).
  • Experienced mental health practitioner.
