Founding Engineer
Our client is looking for a Founding Engineer with 2–6 years of experience to join a four-person team building the infrastructure that makes healthcare billing deterministic. They just announced $100M in committed funding and a design partnership with a $12B RCM company — this is a rare chance to shape the technical foundation of a company tackling a $200B+ market from day one.
The Role
- Building the data pipeline that extracts structured clinical rules from hundreds of thousands of payer policy PDFs (5–4,000 pages each) using LLM-powered extraction
- Architecting and scaling the API and MCP endpoints that serve computable coverage rules at low latency to millions/billions of claims
- Converting medical necessity criteria into FHIR queries and formal logic for deterministic evaluation against patient records
- Taking end-to-end ownership of 0-to-1 projects — identifying high-leverage gaps and building solutions without being asked
- Experimenting with AI coding workflows, agents, and new tooling as part of a lean, high-autonomy team

Key Requirements
- 2–6 years of production backend or data engineering experience (post-graduation, not internships)
- Strong Python skills — significant work in lower-level languages (Rust, C/C++) is a plus as a signal of engineering depth
- Degree in computer science, engineering, or mathematics
- Ability to work on-site 5 days/week at the Hoxton, London office
- UK right to work or an existing visa that can be transferred (no new visa sponsorship)
