Job overview

Are you an experienced and highly skilled clinician with a passion for improving outcomes for people living with frailty? We are seeking a motivated Advanced Clinical Practitioner (ACP) to join our dynamic Integrated Frailty Liaison Team.

This is an exciting opportunity to play a key role in delivering high-quality, patient-centred care across the Frimley Health and Care Integrated Care System (ICS), supporting the most vulnerable members of our community.

This role is fixed term until June 2027.

About the Role

As a Frailty ACP, you will spend the majority of your time (minimum 90%) in direct, autonomous clinical practice, managing a cohort of patients with complex frailty needs. You will work across organisational boundaries to provide seamless, coordinated care that promotes independence, prevents unnecessary hospital admissions, and supports timely discharge.

You will also provide clinical leadership and support to colleagues, helping to shape and develop the service in line with best practice and local population needs.

Reasons to use Rodeo

I’m in my final year doing Economics and I don’t know whether to apply for grad schemes now or do a masters first. What do you think?

Honest answer — it depends on where you want to end up. A lot of top grad schemes (Big 4, civil service, banking) don’t need a masters. Let’s look at the ones you’d be competitive for now, and we can decide if a masters actually adds anything.

Also worth knowing: most autumn 2026 applications are open now. Timing matters more than you think.

Start with a chat, not a search bar

Grad scheme, placement, apprenticeship? Not sure what you want yet — that's fine. Your agent talks it through with you and turns "I have no idea" into a shortlist.
