Background

Care home medicine done properly: with time, continuity and a genuine multidisciplinary team around you. This purpose-built service was created to bring specialist clinicians together to support older people with complex needs across Weston-Super-Mare. It offers an established GP with a passion for frailty, elderly and palliative care the chance to practise thoughtful, relationship-based medicine, away from the pressures of a conventional list-based role.

Location

The role covers care homes across Weston-super-Mare, a substantial North Somerset town with good transport links. Bristol is around 30 minutes by car, Bridgwater around 25 minutes, and Clevedon around 15 minutes, making the role accessible from a wide catchment area.

The Practice

Part of a purpose-built care home hub bringing together other Frailty GPs, a dementia specialist nurse, mental health nursing links, clinical pharmacists and regular geriatrician input under one roof. Recognised by NHS England as a winter pressures exemplar and shortlisted for a national integrated care award for genuine partnership working across organisational boundaries. Led by an experienced, very well-respected GP acting as clinical director who has spent recent years focused on the care of older people and those with complex needs, and who is committed to supporting care home staff so residents receive the best possible care.

Your Role

  • Some remote working available alongside hub-based and care home sessions.
  • Hold an allocated caseload of care home residents, providing proactive and responsive assessment, advance care planning and continuity for patients, families and care teams.
  • Work as a senior clinical decision-maker within a true multidisciplinary team, providing supervision and support to advanced clinical practitioners, pharmacists and nurses.
  • 4-6 sessions over 2-4 days, weekday and in-hours only, with no evenings, weekends, ordinary practice or duty-doctor commitments.
  • Working pattern to include a Monday or Friday, though other patterns will be considered.

Salary And Benefits

  • Circa £11,400 per session
  • NHS Pension Scheme
  • Indemnity covered
  • Six weeks’ annual leave plus one week’s study leave, pro rata, subject to contract
  • BMA contract terms

You must be qualified as a General Practitioner and registered with the GMC and UK performers list.
