The OPAL team works across Wythenshawe Hospital to support with the identification and assessment of frail older adults.

The role of the frailty nurse has rapidly expanded since the first post was created in 2015. This dynamic job requires an individual who can respond to challenges, identify new opportunities and shares the team's passion for delivering best care for frail older adults.

We are looking for a proactive trained nurse who has experience of working with frail individuals. The successful applicant will be supported to expand their skills and knowledge base and will be expected to utilise this in day-to-day practice.

  • Provide a proactive role within the OPAL service to identify and support with the holistic assessment of frail older adults from the front door to discharge.
  • Manage patients on the hospital at home service with support of the wider OPAL MDT.
  • Support with clinics and the development of further ambulatory services.
  • Provide frailty reviews of older adults across the hospital site and support with development of out-reach services.
  • Support the Older Person's Assessment Unit (OAU) including attending consultant ward rounds and assisting with discharge planning and patient flow.
  • Act as a link between the MDT, patient and their relatives / carers and utilise excellent communication skills to promote collaborative working.
  • Deliver planned education across the wards and share specialised knowledge of frailty to colleagues, patients and relatives.
