Role Overview
The successful candidate will be part of the frailty assessment unit consisting of an ambulatory unit providing direct conveyance from community and accident and emergency with a clinical frailty score between 4 and 7. This is a 7 day service from 07:30 to 20:00.
This is a multi-disciplinary approach to assessing and promoting frailty service within the organisation.
The successful candidate will also be expected to rotate to a 12 bedded in-patient frailty assessment unit that consists of a 72-hour stay unit that supports our assessment area and provides comprehensive geriatric care to over 65 year old patients.
Responsibilities
- Ensure they provide care and compassion
- Ensure they maintain the NMC code of conduct and behaviour in a professional manner at all times
- Perform to the job description of a Band 5 Registered Nurse
