Detailed Job Description And Main Responsibilities

What You’ll Be Doing

• Leading and coordinating PSIRF‑aligned patient safety investigations across the Division.
• Applying systems‑based methodologies such as SEIPS, Human Factors, and Just Culture.
• Conducting interviews, evidence reviews, timelines, and systems analysis.
• Producing high‑quality investigation reports with clear findings and proportionate recommendations.
• Ensuring compassionate, transparent engagement with patients, families, and carers.
• Supporting staff involved in incidents and promoting psychological safety.
• Presenting findings at governance meetings and contributing to Trust‑wide learning.
• Working with clinical and operational teams to embed sustainable safety improvements.
• Providing training and coaching on investigation methods and systems thinking.
• Supporting wider Quality Governance activity when required, including tracking actions and identifying themes.
• Deputising for the Head of Quality Governance when needed.

Qualifications

• A Clinical Qualification/registered healthcare professional -desirable.
• Educated to at least degree level-essential.

Knowledge and Experience

Essential criteria

• Understanding and knowledge about PSIIs and PSIRF
• Experience of working in a health or social care setting
• Knowledge of MH services and MH Law – desirable
• Evidence of working with service users and carers to improve safety and quality of care.
• Skills and experience in data management and analysis
• Trained patient safety incident investigator with proven track record in undertaking investigations
• Experience of working with sensitive or distressing information
• Experience in co-ordination of multiple tasks and processes
• Evidence of training in Quality Improvement Methodology or equivalent experience gained working on improvement projects
• Understanding of the Quality Governance Agenda
• Understanding of the regulatory framework under the Health and Social Care Act 2008 and fundamental standards 2014.
