Job Summary
This is a full-time role, designed to bring a strong primary care clinical voice with expertise on managing respiratory conditions in the community, into the programme while supporting delivery at pace. The post holder will support the development and practical operation of integrated respiratory pathways across primary care, Solihull Community Respiratory Team, Virtual Ward, Locality Hub, frailty services, diagnostics, and acute respiratory services. The role is intended to support a left shift in care by enabling more people with respiratory deterioration or complex respiratory need to be managed safely and proactively closer to home, where clinically appropriate.
Main Duties, Tasks & Skills Required
The post holder will also support the Locality Hub, frailty assessment unit interface, and respiratory support offer, working across both reactive pathways for deteriorating patients and proactive pathways for risk-stratified patients. This will include facilitation of SCRT care through investigation requests, testing, diagnostics, treatment optimization, and pathway coordination, in conjunction with SCRT management and respiratory consultant support.
The role will operate at Place level, working in partnership with system leaders, clinicians, PCNs, GP practices, community respiratory teams, acute respiratory services, Virtual Ward, the Locality Hub, and the Solihull Neighbourhood Coach to influence the design, delivery, and embedding of respiratory pathways that support admission avoidance, earlier intervention, and improved outcomes for local populations.

Job Description
- Respiratory clinical expertise -- able to provide credible GP leadership for respiratory medicine, including asthma, COPD, bronchiectasis, breathlessness, diagnostic uncertainty, exacerbation prevention, and long-term condition optimization.
- Admission avoidance and virtual ward working -- able to support respiratory Virtual Ward Admission Avoidance pathways, complex case discussion, and safe care within the UCR/VW ceiling of care.
- MDT leadership and facilitation -- able to contribute effectively to twice-weekly MDTs for complex respiratory admission avoidance caseloads, supporting clinical reasoning, escalation, and shared decision-making.
- SCRT facilitation -- able to work with Solihull Community Respiratory Team management to support care delivery through investigation requests, testing, diagnostics, treatment optimization, and pathway coordination.
- System working -- able to work across organizational boundaries and build effective relationships across primary care, SCRT, UCR, Virtual Ward, Locality Hub, frailty services, acute respiratory services, diagnostics, urgent care, local authority, and voluntary sector partners.
- Pathway redesign -- able to contribute to designing integrated respiratory pathways that improve access, reduce variation, support the left shift of care closer to home, and improve patient outcomes.
- Diagnostics and clinical quality -- strong understanding of respiratory diagnostics such as spirometry, FeNO, imaging, blood tests, and blood gases, and the ability to support safe, standardized use within community and primary care settings.
- Influencing and persuasion -- able to operate at Place level and influence decisions without direct authority.
- Communication and engagement -- excellent interpersonal and communication skills, with the ability to bring people with you and represent both the primary care and respiratory patient voice clearly.
- Quality, safety, and governance focus -- able to work within agreed governance arrangements, recognizing the consultant role in escalation and specialist oversight for complex respiratory cases.
- Pace, credibility, and judgement -- able to work confidently in a complex environment, maintain momentum, and provide clarity where there is ambiguity.

Qualifications
Essential:
- Full registration with GMC with a licence to practise
- On the GMC GP Register
- Evidence of additional training, competence, or substantial clinical experience relevant to respiratory medicine in primary care or community settings
