Join Our Home First Team - Specialist Physiotherapist, Band 6
Are you a passionate Physiotherapist looking for an opportunity to make a real difference to patients as they transition from hospital back home?
An exciting opportunity has arisen for a Band 6 Specialist Physiotherapist to join our Home First Team, supporting patients across Brent and Harrow.
Working within a supportive multidisciplinary team, you will provide rapid assessment and short-term, intensive rehabilitation for patients with complex health and social care needs. You will play an important role in supporting safe and timely hospital discharges, helping patients regain their independence and continue their recovery in the comfort of their own home.
You will manage a varied and complex caseload, working closely with health and social care partners across Brent and Harrow to deliver coordinated, responsive and truly person-centred care.
Our Home First service operates 7 days a week, 8:00am-8:00pm, including bank holidays, with full-time hours worked flexibly across early, late and long-day shifts.

Main duties of the job
You Will:
- Provide specialist physiotherapy assessment and develop evidence-based treatment plans for patients with complex, frailty-related conditions and multiple co-morbidities.
- Independently manage a varied clinical caseload, using advanced clinical reasoning and person-centred goal setting to maximise mobility, independence and support safe, timely discharge home.
- Work collaboratively within the multidisciplinary team and with CLCH specialist services, health and social care partners to deliver coordinated, integrated care.
- Identify and manage clinical risks appropriately, including lone-working considerations, informed consent and timely escalation of clinical concerns.
- Assess, prescribe and order appropriate specialist equipment to support safe mobility, moving and handling, and independent living at home.
- Contribute to the day-to-day running of the service, including triaging referrals, participating in/chairing multidisciplinary meetings and responding to changing service demands.
- Provide clinical leadership, supervision and education to junior colleagues, support workers and students, supporting their ongoing development.

- Diploma/degree in Physiotherapy
- Current HCPC registration
- Evidence of up to date CPD portfolio
- Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation
- Member of Professional Body

- Member of a Special Interest Group relevant to this post
Experience
Essential
- Experience post registration as a Band 5 physiotherapist
- Experience working at a junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory
Desirable
- Experience working with voluntary organisations.
- Experience in service development projects.
Skills & Knowledge
Essential
- Skills in the assessment and treatment of adults with complex multi-pathologies
- Knowledge and understanding of complex medical conditions including multipathologies and falls
- Knowledge of role of Allied Health Professionals and interdisciplinary team working
- Ability to apply core Physiotherapy skills across a range of clinical conditions within a client centered, goal setting
- Knowledge of outcome measures and evidence-based practice
- Ability to problem solve, prioritise and demonstrate clinical reasoning
- Good communication and report writing skills, in both verbal and written format
- Basic IT skills - Microsoft Word, Power point, Excel
- Teaching of junior staff / students, peers in Inservice training setting
- Knowledge and understanding of the role of Social Services and voluntary agencies
- Involvement in Appraisal Scheme
